The program enables residents to access personalised rehabilitation and allied health support without leaving their home environment.

HOMESTYLE  Aged Care has expanded its partnership with Concentric, recently opening its award-winning specialist onsite rehabilitation service at Belmont Grange in Grovedale, with the service also set to open at Sea Views Manor in Ocean Grove by the middle of this year.

Sea Views Manor will become the second Homestyle Aged Care residence in the Geelong-Surf Coast region to offer Concentrics’s rehabilitation program, following the successful implementation of similar services across the organisation.

The service includes a purpose-built specialist rehabilitation gym staffed by qualified physiotherapists and occupational therapists. Delivered onsite, the program enables residents to access personalised rehabilitation and allied health support without leaving their home environment.

Homestyle Aged Care said the expanded partnership reflects its ongoing commitment to high-quality, person-centred care and to improving health and wellbeing outcomes for residents throughout the region.

Concentrics’s services are tailored to each individual resident to achieve outcomes such as increased social participation, increased strength, mobility and balance, and most importantly the program is proven to decrease functional decline.

One example of the program’s impact is resident Nellie, from Homestyle’s Sunset Views Manor in Tarneit, who arrived requiring assistance to stand and unable to walk after suffering a broken neck.

Wearing a neck collar, Nellie set her first rehabilitation goal as being able to walk independently again.

The service includes a purpose-built specialist rehabilitation gym staffed by qualified physiotherapists and occupational therapists.

After several weeks of gentle rehabilitation, Nellie progressed to walking approximately 60m with the support of a walking frame, moving from her bedroom to the gym with encouragement from the physiotherapy team.

She then set a second goal of independently getting in and out of her daughter’s car so she could spend more time in the community. Nellie has since achieved this goal and is now able to transfer in and out of a car with minimal assistance. She no longer requires a neck collar or physiotherapy supervision for walking.

Nellie now regularly walks to the gym independently and uses the exercise bike in her own time while listening to her favourite music.

Her progress highlights the benefits of onsite rehabilitation services in supporting residents to regain mobility, confidence and independence.
