Country to Coast is Surf Coast Shire\u2019s own local, approved provider of the federal government Department of Health\u2019s Home Care Packages.<\/h3>\n

CTC was created to offer the highest quality of care for a reasonable price for people over the age of 65 years who have been assigned a government funded Home Care Package living in Anglesea, Bellbrae, Bells Beach, Jan Juc and Torquay.
\nThese areas have been challenging for organisations outside of the Surf Coast Shire to provide appropriate care services because of higher travel costs for staff. By employing local people with the highest level of skill including three registered nurses and our own trusted carers, we are able to provide professional, trusted supports to meet all of our clients\u2019 needs.
\nWe bring extensive experience and specialised knowledge of the complex Home Care Package industry and combine this with our local focus to ensure communication and quality care outcomes that are unique to each of our individual clients and their individual circumstances.
\nOur approach is to better connect older people and to provide support where and when they need it. It is giving people the time they need by providing staff who have the time for them. We have had some amazing feedback from our clients which has included the improvement to cognition, mobility, general health and mental health. We often talk about the flow on effect \u2013 our care staff create a care plan with clients to work toward achieving goals. When our clients reach these goals, we often find that other areas of their health and well-being improve. By re-assessing care plans together and moving the goal posts, our clients continue to improve enhancing their quality of life at home.
\nMy background is within health and the community services having worked for public and private hospitals, a not profit organisation and the Surf Coast Shire Council.
