Jo and Sue founded Care Pathway Navigators to help people secure funding for care.

“MY name is Jo, and I co-founded Care Pathway Navigators to address the need for a service that assists people in applying for funding for care, enabling them to remain independent in their homes and communities.

“I am a registered nurse with over 35 years of experience.

“I have spent much of that time working in the aged care sector and I have a wealth of knowledge concerning the special requirements of the older members of our community.

“I also have specialised training in paediatrics, caring for children and their families with complex needs.”

Sue recently partnered with Jo in her business.

“I have a wealth of experience managing clients in the human services and health sectors,” Sue said.

“I have been fortunate to work with families and children with high support needs and complex conditions who struggle to access the NDIS.

“During my 30 years-plus experience I have developed expert knowledge enabling me to access suitable support for these clients.

“At Care Pathway Navigators our goal is to reduce the stress of finding care services by managing all of your individual needs.

“With our combined experience of over 60 years in the aged, paediatric and disability sectors, we understand the challenges individuals and families face when attempting to access care and support services.

“Navigating this process can be overwhelming, which is why we offer high-quality, personalised support with a client and family-centred approach.

“We recognise that each journey is unique.

“Our dedicated team will guide, advise, and advocate for you every step of the way to ensure the best possible outcomes.

“We do not provide in-home care services; rather, we navigate the process of finding care.

“This includes assistance with applications, completion of forms for Services Australia, shortlisting service providers, attending assessments, finding suitable care facilities if needed, following up on application progress, and managing other administrative requirements.

“Our aim is to reduce the stress and time involved in accessing these pathways for our clients.

“Many of the families we support are overwhelmed trying to access aged care, DVA and NDIS, and we understand the complexities of this process.

“We write a detailed clinical picture for you, which will enhance your chances of being approved for government funding, and we follow up with all stakeholders regularly to ensure the timely application of care services.

“At Care Pathway Navigators we know how to navigate the systems to get the best outcome for our clients – we know the questions to ask

“We are local to Bendigo, however we service rural and regional Victorians and we provide this service face-to-face, over the phone or using other technological communication tools such as Zoom.

“We offer flexible options that ensure everyone can find a solution that fits their budget and requirements from entry-level assistance to applying for permanent accommodation.

“We also have consulting and navigation services, including regular meetings to review existing services, identify ongoing needs for future planning, keep you informed of changes to funding and services, and discuss and plan for unexpected situations related to declining health and function.

“We aim to empower you to make confident decisions about your care journey.”
