AT Achieve Your Best (AYB), we understand that receiving a diagnosis of early onset dementia or Alzheimer’s disease can be overwhelming for both individuals and their families.
Our experienced and compassionate team is dedicated to providing personalised supports that help people maintain their independence, dignity, and quality of life throughout every stage of their journey.
For individuals diagnosed before the age of 65, support may be funded through the NDIS. Importantly, reaching 65 does not automatically mean a transition to aged care.
Many people choose to remain on the NDIS, allowing them to continue receiving individualised supports in a familiar and comfortable environment.
AYB can support you and your family to navigate this journey while providing an alternative to traditional aged care or group home settings.
Our experienced staff currently provide specialised dementia and Alzheimer’s support, including 24/7 care where required.
We recognise that support needs often change over time, which is why our services can begin gradually with a few hours of assistance each week and increase as needs evolve, right through to full-time or around-the-clock support.
Our supports include:
